Registering with the surgery
To register with our practice please follow the link below to complete the online registration form:
- Register with a GP surgery: Stanley Corner Medical Centre
- New patient health questionnaire form (PDF)
After you have completed and submitted the forms and you are over 16 years old, you will be asked to attend the surgery for the new patient health check.
To help us correctly register you, we might ask you to provide one of the below forms
of identification:
- Passport
- Birth Certificate
- ID Card
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Out-of-practice boundary area registration
Where it is clinically appropriate and practical to register, we now accept new registrations from patients outside our registration area. Patients registered this way would not be entitled to a home visit from the practice. However, if you live outside the practice area and need a home visit, please contact NHS 111 to be seen by a practice closer to where you live.

Temporary residents
If you have visitors who need urgent treatment we may be able to see them as temporary residents. Please ask at reception.
Safe surgery
We are proud to be a safe surgery for everyone in our community and pledge to ensure that everyone in our community receives the quality healthcare they are entitled to.
In partnership with Doctors of the World UK, we will ensure that our practice offers
a welcoming space for everyone who seeks to use our services. We will ensure that a lack of identification, proof of address, or immigration status does not prevent patient registration.
